5. Ship to Party

6. Manufacturer or Supplier (Name and Address)

7. Country of origin

8. Commodity HTS Six Digit Number

As soon as possible, but not later than twenty-four hours prior to arrival:

9. Container Stuffing Location

10. Consolidator (Stuffer) Name and Address[2]
